Procedure for Student Physical Exams for Athletic Teams

The following sports physicals information is for the 2008-2009 school year.  Please check the dates carefully once you view and print the documents. Please download the information by clicking on the following links:

NOTE: Appointment times for June sports physicals for current eighth graders are now posted at Salk School outside the Nurses’ Office and in the cafeteria.  Students should note their scheduled time.  Boys are scheduled for Friday, June 20. Girls are scheduled for Wednesday, June 25.  Students should report to door #16 of the Main Campus of OBHS.  Parents do not need to attend.  If parents have any questions, they can contact the high school nurses at 732-290-3918.
